New Practice Members at Origin Health & Wellness
When you walk into Origin Health & Wellness, you won’t feel like a stranger. Our team is always ready for you, greeting you with a smile just like we would a friend or family member. Our practice members love being here so much that sometimes, they invite other practice members to see a doctor ahead of them, just so they can have more time to chat with others.
Origin Health & Wellness’s atmosphere is one of community, where you feel right at home. Everyone here is going to make you feel cared for, loved and seen.
To prepare for your initial appointment, please bring your identification with you. The forms you’ll need are available on our website and can be brought in with you completed.
ADULT FORMS
You’ll be taken on a quick office tour so you know where everything is. We’ll go over your history before you meet your chiropractor, who will perform a physical examination of your spine. You’ll learn more about what we do, then have a full set of spinal X-rays taken (unless contraindicated).
We’ll wrap up by having you schedule a follow-up, where we’ll detail your findings and our recommendations.
Before you arrive, all three of our doctors have gathered to look at your case. You’ll get the benefit of our combined training and expertise. Our doctors have sat down together to come up with a plan of care that best suits you.
It will be presented to you, along with your X-rays and findings from your examination. All financial information will be discussed, and you’ll have the opportunity to begin care.
You’ll be in and out in 15 minutes or less on a typical appointment. Every 12 visits or 30 days, you will be reassessed, with those appointments taking just a little longer.
Insurance Information
We are not contracted with insurances. We do, however, give our patients super bills so they can be reimbursed directly by their insurance. Our doctors are providers with Medicare. If you have Medicare coverage, be sure to bring your card in with you.
